1) Community Wellbeing Development Opportunities geared towards reduction in infant mortality and growing healthy families, 2) Community Employment Training and Apprentice opportunities for residents’ help address labor shortages and offer young people an opportunity to enter the workforce. and 3) Community Based Life Coaching Training for Community Based Organizations by way of 14-week cohort learning experience inclusive of executive coaching sessions to advance personal, organizational and community leadership skills who provide support and assistance to residents in creating action plans and navigating all of the social determinants of health. These three tiers tied together create a coordinated response in our continuum of care for the County’s most vulnerable residents.
