5.2.1 Logistical Requirements a) Vendor shall provide, at minimum, twenty-five (25) 100 cubic yard walking-floor trailers with trucks and drivers within seventy-two (72) hours of request. b) Coordinate with the SME and NCDA&CS to determine appropriate equipment specifications (specifications such as 100 cubic yard walking-floor trailer). c) Provide licensed operators and certified transport vehicles for the acquisition, transport, and delivery of CSM. d) Provide vehicles and equipment that comply with all applicable federal, state, and local rules and regulations. e) Obtain certified measurements and provide documentation to meet the Federal Emergency Management Agency capacity verification requirements for each trailer used for operations. f) Provide trucks and trailers equipped with a placard on the driver's side of the hauling container: • The placard shall state the vendor's name, an individual and unique identification number, and the total capacity in cubic yards of the hauling container; • Vendor agrees to furnish these placards; and • All placards shall be removed prior to performing work other than activities associated with this contract g) Must use vendor-owned equipment or contracted equipment appropriate to perform required services. h) At the Vendor’s expense, secure and provide all necessary personnel required to perform the services under this Contract. Such personnel shall not be employees of NCDA&CS. i) Cover any incident-related expenses such as lodging and meals for vendor’s employees and subcontractors. j) Invoice NCDA&CS for these services based on the pricing detailed in the Attachment A Pricing Form. 5.2.2 CSM Requirements a) Locate CSM suppliers that have the quantity and type of CSM needed to meet the specifications determined by the SME. Vendor shall factor in the cost of transportation, including locations of the CSM source and destination in determining best price. b) Adhere to any operations and documentation requirements of the CSM supplier, and ensure suppliers adhere to any incident-related documentation requirements. c) Obtain documentation to substantiate the pickup, origin, quantity, type, delivery, and destination of CSM. This documentation may be subject to verification by a third party. d) Coordinate with the SME to deliver CSM to specific drop points at predetermined locations. e) Ensure the required quality of the CSM as defined by the SME continues to meet the specifications as determined by the SME and IMT. The SME and IMT reserve the right to reject CSM that does not meet specifications. f) Upon request by IMT, within 24 hours provide documentation such as global positioning system (GPS) route map, telematics data, or other as specified to verify transportation routes and cost to the IMT. This documentation may be subject to verification by a third party auditor. 5.2.3 Incident Coordination a) Coordinate with the NCDA&CS Incident Management Team (IMT) to establish temporary, co-located office space for a base of operations for incident vendors. The cost of space shall be shared among incident vendors. b) Maintain required, incident-specific biosecurity, as specified by IMT, throughout provision of services.
