Summary of Specifications:
Installation of Curb Ramp, Removal/Replacement of Sidewalk, Curb and Gutter: The City of Jacksonville is requesting bids for the installation of eleven (11) curb ramps along New Bridge Street that will be installed at locations shown on the plans. In addition, ninety (90) feet of 30 inch curb and gutter and thirty (30) feet of 5 feet by 5 feet sidewalk will be removed and replaced. Please submit a unit price for curb ramp construction and a linear foot price for curb and gutter and sidewalk. The removal and replacement of any asphalt will be included in the bid quota. The work to be performed shall include but not be limited to the following elements: 1. Saw cut a minimum of 24” in front of the curb and gutter. 2. All spoils removed during excavation will be removed from roadway within 24 hours. Any spoils that create a hazard on the roadway must be removed immediately. 3. The contractor will be responsible for setting grade to ensure proper drainage. 4. The subgrade shall be back-filled with ABC gravel throughout and compacted to 90% density. 5. Forms shall be of wood or metal, straight and free from warp and of sufficient strength to resist springing during the process of depositing and consolidation of the concrete and shall be securely staked and braced true to line and grade. 6. Concrete class “A” 3,000-PSI will be used. 7. Expansion joints will be used to tie new concrete. 8. Handicap Ramps shall be installed in accordance with City of Jacksonville Specifications detail 405.01 with the only exception the detectable warning mat. The approve mat for use is the “Cast in Place Composite Tactile Mat” in yellow. The size is 2 foot by 3 foot. Theses mats are available through local distributors only. The local distributor is Guaranteed Supply in Wilmington, NC. The phone number is 910-762-6222. . The truncated domes are to be set flush with the concrete. Concrete for Ramps shall be battleship gray. 9. All concrete will be cured with an acceptable curing compound. 10. Contractor is responsible for protecting and guarding newly poured concrete from graffiti from passersby until the concrete has sufficiently cured. Failure to prevent graffiti, or such other vandalism, shall result in the new concrete having to be removed and replaced at contractor expense. 11. Protect new concrete from pedestrian traffic for a minimum of 24 hours and do not open to pedestrian traffic for the first 5 days. Erect and maintain warning signs, lights, and watchmen to protect pedestrian and to direct traffic as needed. 12. The contractor is responsible for setting up the work zone in accordance with the manual of uniform traffic control devices for street and highway construction. Any work zone not properly set-up will be cause for work stoppage by the street’s inspector. 13. All personnel will comply with OSHA standard Personal Protection Equipment, wearing of type two safety vest, steel toe boots and hard hat. 14. The contractor shall provide the City of Jacksonville with Certificate of all insurance. The certificate of insurance will include the City of Jacksonville as additional insured and an original certificate must be returned prior to start of work. 15. Contractor will have a minimum $2,000,000.00 of General Liability and $2,000,000.00 of auto liability. 16. The contractor must obtain a city of Jacksonville privilege license before works begin. 17. NC State Tax: North Carolina Sales tax shall be included in the contract price. The contractor shall maintain and be responsible for seeing that his/her subcontractors maintain accurate records of all payments of North Carolina State taxes on materials, supplies, fixtures, and equipment which become part of or are annexed to buildings or structures, including roads, sidewalks, and pipelines provided under this contract.
18. Request for payment: the contractor shall submit with his/her request for payment, a certified statement showing such payment by him/her and all subcontractors listing invoices from various suppliers showing invoice number, amount paid and date. Sales records ant) certified statements shall be in such form and substance as to meet the requirements of the North Carolina department of revenue in the matter of the city obtaining a refund from the state of North Carolina sales taxes paid by the contractor and his/her subcontractors. 19. Right to terminate contract: if in the judgment of the owner, the contractor is not capable of performing the desired work in accordance with the contract documents either because of financial reasons, failure to supply necessary skilled workmen, unsatisfactory tools or equipment, inadequate supervision, or revocation of the contractor's license, the owner shall have the right to cancel or terminate under this contract by giving the contractor seven (7) days written notice. 20. Controlling law: any agreement, contract, or purchase order resulting from this invitation, request for bid, quote, or request for proposal shall be governed by the laws of the state of North Carolina. 21. Any additional work related to this project will be performed by the contractor only upon the prior written request of the street division and concurrence of the contractor. Any additional work to be performed will be approved by the street division using the city's contract/purchase order change request.
