FIBER OPTIC RECEIVER AND TRANSMITTER WITH INSTALLATION, ON-SITE TESTING, LEVEL ADJUSTMENTS WITH MASTER CONTROL AND CERTIFICATION AND FOLLOW– UP VISIT ON SPECIFIC DATE AND TIME
I. Quotes Due All quotes are due on 9/21/2015 by 9:00 am for the purchase of MEDIA ELECTRONIC EQUIPMENT WITH INSTALLATION, ON-SITE TESTING, LEVEL ADJUSTMENTS WITH MASTER CONTROL AND CERTIFICATION AND FOLLOW – UP VISIT ON SPECIFIC DATE AND TIME. Interested bidders must sign this Request for Quotes and either email it to astewart@jacksonvillenc.gov or deliver to Angela Stewart at the Finance Division at Jacksonville City Hall located at 815 New Bridge Street, Jacksonville, NC 28540.
II. Qualifications The successful bidder must be properly licensed to do business in the City of Jacksonville. The City is exempt from Federal Excise Tax. Our Federal Tax Identification Number is 56-6000232. The City is not exempt from NC State & County Sales Tax. Such taxes shall be shown as a separate item on all invoices.
III. Rights The Jacksonville City Council reserves the right to waive any informalities, to reject any or all bids, and to accept that Bid or Bids which appear to be in the Owner's best interest.
IV. Inspection All materials/supplies shall be the subject of inspection by the Project Manager at time of delivery.
V. Quotes to be Retained Quotes are to remain valid through 10/21/2015.

CITY OF JACKSONVILLE INSURANCE REQUIREMENTS Vendor must obtain and provide proof of the following types of insurance with the dollar limits designated. Additionally, Insurers must be licensed to do business in the State of North Carolina. 1. Worker’s Compensation Insurance: In accordance with North Carolina Statutory limits 2.
