Dear Business Owner,
In celebration of “Older Americans Month”, the Jacksonville Recreation and Parks Department will be hosting a picnic on Wednesday, May 6, 2009 (raindate on May 13, 2009). We would like to give an opportunity for your agency to bid on the following event that will be catered. Please take the time to review the following menu items and consider pending your availability. We will begin accepting bids on Monday, February 23, 2009 at 8:00AM, the bidding process will end on Friday, February 27, 2009 at 12:00PM. • All quotes will need to include tax, gratuities and should be stated as price per person, with grand total indicated separately, • Service for approximately 75 people (minimum 75), • Request full catering service to include serving staff, set-up serving table, food, utensils (spoon, forks, knives and napkins) and cleanup. • If selected, must be able to provide the following for payment processing. Completed COJ Vendor Packet (Vendor Application, W-9, etc.), General Liability Insurance Certificate listing COJ as additional insured, Workers Compensation or Independent Contractor Form (pending number of employees).
