THE STREET DIVISION IS INTERESTED IN OBTAINING QUOTES THAT CAN BE USED THROUGHOUT THE FISCAL YEAR 2006(January 2006 - JUNE 30, 2006) FOR THE FOLLOWING TYPES OF HORIZONTAL CONSTRUCTION WORK
1. CONCRETE WORK PER ATTACHED SPECIFICATIONS

Note: All bids shall include the following:
1) Saw cut a minimum of 12” in front of curb and gutter/valley gutter.
2) Any sod removed from back of curbing will be replaced with same type; the contractor is responsible for watering sod until new growth is established.
3) All spoils removed during excavation will be removed from roadway within 24 hours. Any spoils that create a hazard on the roadway must be removed immediately.
4) The contractor will be responsible for setting grade to ensure proper drainage.
5) Sub-grade shall be uniformly back-filled with ABC gravel throughout and compacted to 90% density.
6) The Street Division’s inspector will inspect the sub-grade prior to pour of concrete. It will be the contractor’s responsibility to contact the inspector at least two (2) hours prior to pour. If rainstorm occurs before concrete is poured the Street Division’s inspector will inspect the sub- grade and determine when pour can be made.
7) Only steel metal forms may be used for valley gutter and curbing gutter, Forms must be in place for 7 days after pour. Wood forms may be used for sidewalk, provided they are straight and free from warp and of sufficient strength to resist springing during the process of depositing and consolidation of the concrete.
8) Concrete will be class “A”. 3,000-PSi for sidewalk, curb and gutter, and crass “A”, 4,000-PSI with fiber for valley gutter.
9) Expansion joints will be used to tie new concrete.
10) All asphalt involving curb and gutter/valley gutter is to be included with quote.
11) All concrete will be cured with an acceptable curing compound.
2. ASPHALT WORK IN ACCORDANCE WITH D.O.T. STANDARDS AND SPECIFICATION FOR:

E. UTILITY CUTS __________ UTILITY CUTS- Work consist of saw cutting area at least a foot into good pavement, removing existing cold patch material, filling area with ABC gravel if necessary, tacking vertical walls and filling 2 inches of hot asphalt mix (9.5A or 9.5B).
3. PROVIDE COST FOR RAISING VALVE BOXES AND MANHOLE FRAMES.
The contractor will be responsible for adjusting all valve boxes and manhole frames and covers to conform to the proposed finished pavement elevation. The adjustment for manhole frames and covers shall be done in accordance with Detail C06.3 (Enclosed). Valve boxes will be raised in accordance with Detail 513.02 (Enclosed).

2. The contractor will have (specified of days or date) from notice to proceed to complete work.
3. Contractor will have minimum insurance policy for general liability of $1,000,000. 00 and auto liability of $ 1,000.000.00.
4. The contractor shall provide City of Jacksonville with certificate of all insurance with the City of Jacksonville as secondary insured (to remain in force during the construction period). The certificate of insurance shall be an original; a copy will not be acceptable.
5. The awarded contractor must obtain a City of Jacksonville Privilege License before work can begin.
6. Any traffic devices such as barricades, traffic cones, etc., left by the contractor during the project will be checked daily and set up in accordance with the manual of uniform traffic control devices for streets and highway construction
7. The street division inspector will provide exact locations for work during initial pre-construction meeting.
8. All personnel will comply with OSHA standard regulations, i.e., personal protection equipment (PPE), safety vest, safety shoes, hardhat etc.
9. No asphalt work will be performed when temperature is below 35°F.
10. No cement work will be performed in inclement weather,
i.e., rain, snow, hurricane, etc.
11. Wheel chair ramps will be installed conforming to City of Jacksonville Detail 405.01(Enclosed).
