Tompkins County Whole Health, in partnership with the City of Ithaca and the Downtown Ithaca Alliance, seeks proposals from nonprofit organizations with proven histories of providing community outreach and care coordination for a countywide Mobile Supportive Services program. The selected provider will be responsible for fully staffing, operating, and managing two teams of community outreach workers who will respond to community concerns in Downtown Ithaca and identified areas of need outside of the city, merging the previous separately funded Community Outreach Worker and Rural Outreach Worker programs together into one program. An annual amount of approximately $395,000 is available through 2026, with the potential for longer term funding renewal subject to legislature approval.
Tompkins County Whole Health will be the lead administrative agency for this project and will provide project vision and direction, project management, and contract administration.
