There are several principles and components specific to the partnership of FACT with TAP that inform the overall process. These include: • The 30-unit TAP will be dedicated to 30 individuals receiving services from the FACT team. • Ensuring service access by managing FACT and TAP referrals through the County Local Government Unit (LGU) Single Point of Access (SPOA) and Human Resources Administration (HRA) systems. • FACT and TAP staff will coordinate the delivery of holistic wrap around services. This requires regular communication between the FACT staff and the TAP staff. TAP staff may attend the FACT team’s daily team meeting to help coordinate efforts for individuals served by both programs. • Collaboration and coordination of services between FACT and TAP staff to ensure behavioral health and restorative services address the unique needs of individuals with SMI who are involved in the criminal justice system. • Providing support and skills training to individuals in a community setting. • After admission to either FACT or TAP, continued enrollment in one program is not contingent on continued enrollment in the other. Meaning, if someone no longer requires FACT, it doesn’t mean they have to be discharged from TAP (or vice versa).
