CONSTRUCTION TASK Provide all necessary work for the upfit of the space leased for the Clifton Park Community Based Outpatient Clinic (CBOC) in Clifton Park, New York. A. Background Information: The Albany VA Medical Center in Albany, NY is seeking to renew and expand an existing lease of approximately 5,000 nets usable square feet to located at 963 Route 146, Clifton Park, NY 12065. The clinic will contain Primary Care, outpatient Mental Health, Telehealth specialty services, laboratory, Hemo infusion therapy, Physical Therapy, and Occupational Therapy services. The space will be upfitted to meet the Community Base Outpatient Clinic design guidance per attached layout drawing and scope of work. B. Project Intent: Provide the Clifton Park CBOC with space that fully supports efficient patient care operation and that allows the clinic to meet its projected workload. C. This project includes all design services required to upfit the expanded square footage as required per attached Clifton Park CBOC Architectural Scope of Work including security provisions dated April 24, 2025, and Clifton Park CBOC Drawing A1 & A2 dated May 15, 2025, for the expanded lease space, including all utilities, to meet the VA s needs and the requirements of this document. D. Weekly meetings and site visits will be conducted with the property owner and VA staff to address any questions and monitor progress of the project scope of work. These meetings will include review of the layout plan, scope of work, detailed discussions of the various project elements. Discussions will also include design constraints, access requirements and all applicable codes and standards. II. Design Requirements A. Property owner is responsible to hire a licensed designer / professional to ensure all mechanical, plumbing, fire protection, HVAC, and electrical codes are maintained due to upfit modifications/changes. B. Licensed designer / professional shall provide all necessary engineering and design services to ensure the entire space maintains appropriate HVAC systems. The property owner is responsible for making all necessary HVAC modifications to maintain all spaces at 70 degrees year-round. In addition, the property owner is responsible for making all necessary HVAC modifications necessary to maintain 30%-60% relative humidity in all seasons with four air exchanges per hour. C. Licensed designer / professional shall provide all necessary engineering and design services to ensure the entire space maintains appropriate plumbing systems. The property owner is responsible for making all necessary modifications to ensure domestic water temperature is no greater than 110F. D. Licensed designer / professional shall provide a phasing plan that minimizes impacts on patient care and workflow. Always maintaining a dedicated women s exam room. E. Licensed designer / professional shall provide all necessary construction documents required to ensure all work is completed in accordance with the attached scope of work and layout drawings. III. Upfit Requirements A. The attached Clifton Park CBOC Architectural Scope of Work including security provisions dated April 24, 2025, and Clifton Park CBOC Drawing A1 & A2 dated May 15, 2025, provide complete requirements for the necessary upfit modification to expand the Clifton Park CBOC. The property owner is responsible for all necessary work. ARCHITECTURAL General Notes Entire space includes complete painting of all walls, doors and trim. Patch and paint all the walls depending on the level of removal and damage with renovation/move. Remove all items hung previously (subject to being replaced). Ensure entire space includes fully functional and operational lights. Damaged lighting fixtures need to be replaced. Accommodate required electrical and data outlets according to floor plan and equipment layouts. New electrical/data outlets must be added within 3 feet of all equipment shown on drawing A1 & A2. Lighting to be modified to new area. 255 General Notes Scope OIT General Notes A reconfiguration of IT infrastructure to meet the requirements of the CBOC upfit to include: - Install VA Wi-Fi as part of CBOC expansion. - Expand network range to encompass additional space. Expand network switch capacity to meet new requirements. - Reconfigure available TIPS to meet the requirements of new/reconfigured spaces. Obtain and install additional IT hardware to provide the required connectivity. - Work with COR and NetOps for organizing extensions/numbers for telephone needs. - Work with HTM and NetOps to configure network settings for additional equipment. - All new data lines are to be CAT6a. SECURITY PROVISIONS The objective of this requirement is to provide the appropriate level of physical security that will, when constructed and implemented, providing the appropriate level of physical security to support the services provided by this clinic. The intent is to protect the safety of the patients, staff and visitors in case of an emergency or incident by the use of physical measures. Measures designed to safeguard people, to prevent unauthorized access to equipment, facilities, material and documents, and to safeguard against damage and loss. The installation, maintenance, monitorship and responsibility for notifying local police of clinic alarm activation should be accomplished through a contractual arrangement with a commercial security firm. The installation shall be incorporated into the Tenant Improvements and the maintenance and monitorship shall be incorporated into the monthly payments. SECURITY SYSTEM: Provide a motion intrusion detection system for the entire VA space that is monitored by motion sensors and contacts on exterior doors and windows, and also a door contact on the IT Room door. The system shall meet the following requirements but not limited to: (1) An internal, automatically charging DC standby power supply and a primary AC power operation. (2) An automatic reset capability following and intrusion detection. (3) A local alarm level of 81dB (min.) to 90 dB (max.) within the configuration of the protected area. (4) A low nuisance alarm susceptibility. Shall not exceed a rate of one (1) alarm per seven days within the first 60 days after installation and acceptance. Sensor adjustments will be made and then not to exceed one (1) alarm per 30 days. (5) Annunciators/keypads shall be located at all entrances and shall have the capability of identifying protected zones. (6) No access permitted from the patient corridor into the Common Work Area. If passage is required in an emergency, the door needs to be equipped with electronic hardware, magnetic locks & connected to the fire alarm system. It shall also have a PIV to allow a staff member to go from the patient corridor into the Common Work Area. (7) Locks shall be Best Lock with removable core cylinders that are removable only with a special key or tool without disassembly of the knob or lockset. Cylinders shall be 6 or 7 pin. The finish shall be 630 stainless steel with antimicrobial coating. (8) System shall have the capability of transmitting alarms to an off-site 24 hr. monitoring company and VA police service. (9) Telecommunications Room: Room must have motion detection and camera inside the room and intrusion detection on the door. (10) Motion detectors shall be installed in the Waiting Area, Conference Room, Corridor (as needed pending the configuration). SPECIAL SECURITY REQUIREMENTS: The VA Police will conduct vulnerability assessments over the course of the lease term to determine vulnerabilities in physical security and provide recommended corrective actions. The clinic shall use a 2-control level system of master, individual and special keys. Lessor shall provide 2 copies of the master, each individual and each special key including 6 blanks for each key provided. The VA is responsible for establishing a keying plan. Keys produced shall not bear identifying marks, such as VA and/or US Government . The loss of theft of VA space keys by non-VA personnel shall be reported as soon as notified to the VA Police/Nurse Manager. If the VA police investigation does not immediately result in the recovery of the key, or a positive determination is made that the loss occurred off facility property, the key cores operable by the lost key will be changed and all affected keys replaced with five days at Lessor s expense. Persons found to be in unauthorized possession of VA keys, or in possession of unauthorized reproduction of such keys, shall be cited by VA Police for the appropriate violation of Title CFR 1.218. Medicine closets shall have a drywall ceiling. Walls and ceilings shall be constructed with a woven mesh, 1.2mm (.047 in.) wire diameter alloy #304 stainless steel, with tensile strength of 29 kg/mm (1,600 lbs./sq. in.) PANIC ALARM SYSTEM: Lessor shall furnish and install one (1) panic alarm system. The system should have a wireless Panic Alarm device in all the exam rooms, reception area, consultation room, conference room and the blood draw room. The panic alarm indicator light to be installed above the patient care room doors and will transmit to the Common Work Area, Reception Area and the local police. ROOM CONDITION INDICATOR LIGHT SYSTEM: An in-use indicator light system to be installed for each exam room providing a visual indication of the activity in the patient care room. The patient may use the system to indicate that he/she has completed gown/ungowning and staff can enter the room. The system shall also inform staff working outside of the room which staff member is currently in the room with the patient. The color of light illuminated identifies either the Provider, Nurse, Health Technician or other staff is with the patient. SITE ENTRANCES: Lighting must be provided at site entrances to provide illumination in support wayfinding, security and safety needs by assisting pedestrians and vehicles to locate their destinations expeditiously.
