Specifications include, but are not limited to: The Monroe County WIC Program proposes to contract with a vendor to provide one (1) Nutritionist and three (3) Public Health Assistants in order to improve the compliance rates for appointments of existing and new WIC clients and to expand and improve WIC outreach. The following services will be provided within this contract period: A. The Contractor will provide one (1) Qualified Nutritionist who will perform the following tasks: 1. Conduct nutrition assessments, determine nutrition risks and enroll participants; 2. Certify/recertify participants, provide nutrition education and authorize the issuance of WIC checks and/or eWIC cards; 3. Develop and implement high risk care plans and oversee high risk care to participants; 4. Coordinate nutrition education, including breast feeding promotion and support that is responsive to the identified needs and interests of each participant; 5. Track participant progress in improving their health and document outcomes; 6. Refer participants to other health related and social service agencies as indicated by their identified needs; 7. Provide oversight and guidance to Nutrition Assistant and Public Health Assistant staff; and 8. Provide leadership in planning, conducting, and evaluating Participant Centered Nutrition Education Services. B. The Contractor will provide three (3) Public Health Assistants who will perform the following tasks: 1. Assist in the development and delivery of educational services regarding WIC’s services to community agencies and health care providers for the purpose of increasing awareness of WIC programs, including FIT WIC, among community providers and facilitating enrollment of new clients from these community providers; 2. Assist with outreach efforts including scheduling appointments, conducting appointment reminder telephone calls, sending follow up letters, emails and text messages, etc. to clients and community based providers; 3. Participate in appropriate data collection per Monroe County, New York State Department of Health (NYSDOH) and Federal guidelines and requirements; 4. Provide WIC site support to the Nutritionists and other WIC staff, including assisting with certification/recertification of participants; 5. Screen participants for eligibility; 6. Obtain immunization and nutrition status; heights and weights; and required lab tests; and 7. Receive and makes phone call to/from participants and the public for the purpose of providing information and scheduling appointments.
