1. Selected Respondents will provide Transitional Housing for use by MCDSS’s Public Assistance applicants and/or recipients. a. Selected Respondents shall place eligible Public Assistance applicants and/or recipients in housing at a facility or building owned or legally authorized to rent by the Selected Respondent. 2. Selected Respondents will adhere to the following process for TH approval: a. Submit a TH application for an individual (or Head of Household for a family) who has not been in another TH, Congregate Level II, or supportive housing program in the previous 12 months. b. Submit a letter of support and landlord statement for each potential admission. The letter of support must specifically note the individual is referred for TH and must be provided by the individual’s service provider (inpatient facility, substance use counselor, parole/probation officer, etc.) and cannot be provided by staff from the proposed TH facility. c. If approved for admission, TH providers will submit the individual’s independent living/service plan to the agency within 10 business days after approval. 3. Selected Respondents will provide Case Management Services to TH residents. Minimum case management services should include the following activities: a. Work with clients to apply for ongoing financial assistance. Assist client in establishing eligibility, follow up to ensure client keeps all required appointments (including but not limited to Job Search, Employment Assessments, Rehabilitation, Physical, Mental Health or Industrial Medical Associates appointments). For clients already active on Public Assistance, work with client to maintain eligibility. b. Complete an independent living/service plan that incorporates the Division's Employability Development Plan (EDP). For clients without a Division EDP, the Contractor will create one. The independent living/service plan must be submitted to the Division at the time the client enters the facility. The Contractor further agrees to monitor each client's progress in complying with the EDP. c. Workshops that include but are not limited to life skills, financial literacy, nutrition, how to be a responsible tenant, and parenting (required for custodial parents only). d. Provide transportation as needed. e. Provide child care as needed. 8 f. Assist clients to find stable, permanent housing at the end of the transitional housing period. Activities include but are not limited to locating housing, setting up appointments to see housing, providing advocacy with landlord and making referrals to Rapid Rehousing as appropriate. Also includes verifying prospective housing is free of any health and safety violations. g. Coordinate moving arrangements out of transitional housing, including but not limited to arranging delivery of existing furniture or procurement of a furniture voucher, facilitating utility turn on, urging clients to choose budget billing, assisting with school registration or address updates as needed. 4. Respondents can propose to serve special populations of clients, including veterans, teen parents, sex offenders, ex-offenders on Probation or Parole, older adults, families or the chronically homeless. 5. The selected Respondent retains the right to determine whether they will accept an individual into their facility. 6. MCDSS retains the right to inspect the building where transitional housing is offered at any time when public assistance recipients are being served under contract. 7. Selected Respondent will enter all reporting into MCDSS’s web-based Contrack HQ contract management system on a monthly basis and will report on the following performance evaluation criteria: a. number of TH residents in care during time period b. number of TH residents who successfully completed transitional period and moved to permanent housing c. number of TH residents who successfully completed substance use treatment d. number of TH residents who became employed
