Specifications include, but are not limited to: 1.1.1 The Maryland Department of Health (MDH or the Department), Prevention and Health Promotion Administration/The Center for Chronic Disease Prevention and Control, is issuing this Request for Applications (RFA) to develop, implement, and evaluate a series of skill-based training sessions focused on assessing and addressing social determinants of health (SDOH) factors impacting diabetes prevention and management and health equity. 1.1.2 It is the State’s intention to obtain services, as specified in this RFA, from an Agreement between the selected Applicant and the State. The anticipated duration of services to be provided under this Agreement is 6 months. 1.1.3 The Department intends to make a single award as a result of this RFA. 3.2.2.4 The training objective is to increase the capacity of leaders, managers, lifestyle coaches, and educators to assess and address factors related to the social determinants of health (SDOH) and health related social needs (HRSN) impacting the ability of people with and at risk for diabetes to successfully access and engage in evidence-based care and services. Therefore, the Applicant shall develop a series of training opportunities on the following topics: Topic #1: Raise Awareness and Understanding: Provide participants with a foundational understanding of SDOH and HRSN and their impact on diabetes prevalence, incidence, and outcomes. This module will emphasize the interconnectedness of social, economic, and environmental factors. Topic #2: Assessment and Integration: Equip participants with practical tools and methodologies for assessing SDOH and HRSN within their patient populations and community settings. Emphasize the integration of this information into diabetes prevention program planning, implementation, and evaluation. Topic #3: Cultural Competency, Cultural Humility and Tailoring Interventions: Foster cultural competency and cultural humility to ensure interventions are tailored to the unique needs and challenges of diverse populations. This module will explore strategies for engaging communities and adapting interventions to different sociocultural contexts. Topic #4: Advocacy and Collaboration: Empower participants to advocate for policy changes and collaborate with community stakeholders to address systemic barriers contributing to health disparities related to diabetes. This module will focus on building partnerships and leveraging resources to create sustainable, community-driven solutions.
