The intent ofthisfunding isto provide outreach,screening, and referralservicesto older adults(ages 60 and older) who are living independently or with caregivers in Monroe County and who experience mental health-related symptoms. Services that should emphasize, at a minimum, the following services to assist clients in successfully maintaining community living: 1. Outreach to identify older adults with untreated mental illness; 2. Referrals to treatment and prevention services; 3. Coordination of mental health, social service, and primary health services, including consulting with client providers; and 4. Education and support for caregivers of older adults. Senior screening services can be delivered in the community in any setting in which people may benefit from education and outreach, including schools, medical providers, hospitals, religious settings, criminal justice settings, places of employment, social service providers, and drop-in centers. This would not include existing mental health treatment settings or developmental disability programs, as participants are already receiving services. Senior screening services should be provided with cultural and linguistic competence and from a trauma-informed perspective. Please note that this RFP does not include NYS OASAS-funded Prevention services.
