• Range of Motion (ROM) program daily • Ambulation program • Quarterly bed mobility assessments • Initial and room change environmental assessments • Zero lift training at orientation • Wheelchair adjustments and modifications as needed • Care plan adjustments per referrals and screens daily • Attend care plan meetings, 48-hour care plan meetings, & discharge planning meetings • Clean and sanitize all utilized equipment daily • Wound round attendance • Assist with meal service, as needed • Assist residents and their families with car transfers • Outside environmental assessments with residents and families to determine independence • Update Resident outside list with Security • Provide initial and ongoing power wheelchair training • Home visits, as needed • Discharge equipment orders for discharging residents • Identifying need for change in seating cushion or in wheelchair systems for resident positioning • Screen all new admissions for correct diet • Address all nursing referrals for diet exceptions and change in quality swallowing • Assess and monitor hand orthotics • Ordering adaptive equipment for resident use • Communicate and schedule with the United States Department of Veterans Affairs (VA) and other external vendors for Adaptive Equipment and ceiling lifts.
