Some of the specific deliverables within the contract, must include, but are not limited to: • Intake with returning individuals; • Case Conferencing in partnership with New York State Department of Corrections and Community Supervision (NYSDOCCS); • Service Planning and Coordination • Connection to the Ready, Set, Work! (RSW!), employment program; • Connection to Moral Reconation Therapy (MRT) in coordination with Westchester Independent Living Center (WILC) and other service connections as need is determined; • Transportation assistance (Metro Cards, etc.) • Identification documentation assistance (Birth Certificates, NYS Identification Cards, etc.) • Conduct Quarterly Task Force meetings. • Connections and referrals to other state agencies that provide specialized services to multi-dually diagnosed individuals such as OPWDD, Office for the Disabled. • Connect and refer individuals to state funded agencies for Immigration Services support • Connect and assist with DSS application pre-release and post-release to expedite and secure the process of obtaining public assistance, shelter, Medicaid or temporary assistance. • Provide welcome home kits and support for community integration • Digital Equity by providing or facilitating cell phones and assisting referrals to the County Library System to familiarize participants with the digital era. • Referrals to community agencies to foster healthy relationships.
