1. Conduct review of historic plans, and recent surveys, incorporate them into preliminary plans and a design report. Report shall include existing conditions, proposed improvements, and constraints. 2. Review existing survey and determine if additional survey work is necessary. Surveys completed in April 2012 and April 2021 can be viewed at the Department of Public Works, 285 Denton Ave., New Hyde Park, NY. Survey services are subject to NYS prevailing wage rates. 3. Location of all underground and overhead utilities. 4. Prepare, file, and secure all necessary permits and approvals. Consultant to identify all required permits and approvals in proposal. 5. Attend up to ten (10) meetings with DPW and Town representatives to review and finalize project elements. Consultant will be responsible for providing material samples, product documentation/brochures to assist the Town in making final selection of proposed features, surfaces, etc. Meetings should include at least one Utility coordination meeting with the local utility companies and Nassau County DPW. 6. Attend up to two (2) public meetings, if required. Consultant will be required to prepare and present renderings of proposed final project elements. 7. Preparation of final design plans to be used as construction bid documents (construction drawings, specifications following the Town’s Boiler Plate format). Final design shall be in strict accordance with all applicable codes and regulations. 8. Preparation of construction cost estimate and construction schedule. 9. Conduction of pre-bid activities up to contract award including but not limited to; answering Contractor Request for Information (RFI’s), clarifications, and evaluation of bid proposals and reference check. 10. Construction administration services. Under this task, Consultant shall assume a minimum allotment of 200 hours, which will include but not limited to; shop drawing/submittal review, attendance of bi-weekly construction progress meetings or as required, preparation of revised contract documents/sketches as required, and regulatory compliance needs. Construction management/inspection services are not included under this task. 11. Consultant shall identify any additional tasks in the proposal required to complete design services, including out of pocket or reimbursable expenses.
