● Provide direct evaluation and Physical Therapy treatment services in compliance with Federal and New York State Education law and regulations, and in accordance with each student’s Individualized Education Plan (IEP), as prescribed by the District’s Committee on Special Education (CSE) or subcommittee of the CSE. ● Provide on-site New York State licensed Physical Therapists and Physical Therapy Assistants to meet the treatment needs of the district. ● Provide timely and thorough Physical Therapy evaluations and reports as requested by the District’s Committee on Special Education (CSE) or subcommittee of the CSE, including initial eligibility evaluations and triennial reassessments using appropriate standardized testing methods and professional observation and data to support recommendations. ● Physical Therapist will provide all required clinical supervision and oversight for evaluations and services provided by Physical Therapy Assistants under this agreement or through other contracts, serving students within the District, as assigned by the Office for Students with Exceptionalities.
