General Occupational Therapy Services to Be Provided: ● Provide direct evaluation and Occupational Therapy treatment services in compliance with Federal and New York State Education law and regulations, and in accordance with each student’s Individualized Education Plan (IEP), as prescribed by the District’s Committee on Special Education (CSE) or subcommittee of the CSE. ● Provide on-site licensed/registered Occupational Therapists and Certified Occupational Therapy Assistants to meet the treatment needs of the district’s students in 7 in-district schools as well as evaluation services for students of the district who are placed out of district. ● Provide timely and thorough Occupational Therapy evaluations and reports as requested by the District’s Committee on Special Education (CSE) or subcommittee of the CSE, including initial eligibility evaluations and triennial reassessments using appropriate standardized testing methods and professional observation and data to support recommendations. ● Obtain all required scripts for treatment and implement services in accordance with IEP recommendations. ● The Occupational Therapist (OT-R) will provide all required clinical supervision and oversight for evaluations and services provided by Certified Occupational Therapy Assistants (COTAs) under this agreement or through other contracts, serving students within the District, as assigned by the Office for Students with Exceptionalities. ● Complete all relevant sections of the student’s IEP including inputting test scores, and completing the present levels of functioning, strengths, needs and narrative descriptions, as well as development, inputting and progress monitoring of appropriate goals in the domain of physical development. ● Review educational records, including communicating with individuals involved in the creation and implementation of each student’s IEP. ● Prepare for and participate in any required legal process as a representative of the school district, including testifying at or providing information for any litigation and/or impartial due process hearings as needed. ● Maintain and submit all documentation in compliance with Federal and New York State requirements and District procedures and policies, including treatment notes, Medicaid billing, and IEP goal progress and observations. ● Provide quarterly written progress reports and annual summaries for student IEP goals through the District’s IEP management system and repository.
