Nyack U.F.S.D. consists of three elementary schools, one middle school, one high school and one non-public school located within our school district boundaries. We anticipate that students will receive Occupational Therapy Services based on the needs identified in their Individual Education Plans (IEP’s). At this time, we anticipate that approximately 45 Occupational Therapy service hours per week will be required. However, throughout any given school year there may be additional students who receive services as part of a 504 plan. Please note that the anticipated needs as presented are estimates. The actual numbers will vary based on recommendations from the Committee on Special Education meetings that occur throughout the school year. Responsibilities of the Certified Occupational Therapist include, but are not limited to: 1. The chosen provider will work cooperatively with the Central and Building level administrators, physical therapists, school psychologists, speech therapists and teachers, and Occupational Therapy Assistant to provide researched-based, educationally appropriate services to Nyack students. 2. In conjunction with a student’s IEP, provide therapy to a student individually or in group sessions (not to exceed five students). 3. Adhere to assigned schedule including dates of service which follow Nyack Public School’s yearly calendar along with the dates prescribed in each IEP, and hours of service determined by the District within each school’s hours of operation for each school in the district. 4. Prepare and submit all required paperwork in an accurate and timely fashion, including but not limited to information related to the development and implementation of a student’s IEP as it pertains to the delivery of services, present levels of performance, goals and objectives and periodic updates on goal achievement to parents, to coincide with report card distribution. 5. Provide basic materials and supplies necessary to conduct Occupational Therapy sessions. 6. The basic rate for Occupational Therapy services (Rate 1) will include screenings, evaluations and reports (including Medicaid), school related meetings (including CSE and Department meetings), and supervision of Occupational Therapy Assistant services.
