The selected Respondent(s) will provide, as needed, one or more full or part time Qualified Nutritionists (Registered Dieticians are not necessary) who will perform the following tasks: 1. Conduct nutrition assessments, determine nutrition risks and enroll participants; 2. Certify/recertify participants, provide nutrition education and authorize the issuance of supplemental foods in eWIC cards; 3. Develop and implement high risk care plans and oversee high risk care to participants; 4. Coordinate nutrition education, including breast feeding promotion and support that is responsive to the identified needs and interests of each participant; 5. Track participant progress in improving their health and document outcomes; 6. Refer participants to other health related and social service agencies as indicated by their identified needs; 7. Provide oversight and guidance to support staff; and 8. Provide leadership in planning, conducting, and evaluating Participant Centered Nutrition Education Services.
