The Contractor will provide primary medical care including, but not limited to the following: a. Conduct initial medical evaluation and obtain basic medical work up for Individuals newly admitted to the specified IRA’s subject to this agreement. b. Conduct annual medical work ups, including physical exams, and other appropriate diagnostic evaluative tests, as needed or required. c. Ensure continued Treatment Provider’s orders for all Individuals’ medications. Provide updates or renewals as necessary. d. Provide ongoing medical care, as appropriate in the above residence(s). For individual who are admitted to Noyes Memorial Hospital in Dansville, NY, follow the individually closely and consult with the Hospitalist formally or informally to aid to their care, including, reviewing of their charts and seeing them in the hospital, as needed.
