The Monroe County Office for the Aging (MCOFA) is seeking an entity or entities to provide efficient and costeffective Information and Assistance including Case Assistance, Case Management for Caregivers, Supplemental Services for Caregivers, and Public Education & Outreach services to Monroe County’s underserved low-income older adults age 60 and over ("Older Adults"), and if supported by the program, an informal caregiver who is 18 and over ("Caregiver"). Services can include: crisis intervention, short-term problem solving such as housing, caregiver support, psycho-social health assessment, "benefits check-up," and other services such as public education & outreach presentations at area senior centers and other locales where Older Adults congregate. Special emphasis must be placed on targeting underserved populations including, but not limited to low- income older adults, adults with limited English proficiency, older people of color, and other groups that may not be currently linked to community-based services. Please refer to 25-PI-01 for further information on NFCSP requirements. The goals of the caregiver related services include: 1. Empowering caregivers in fulfilling their role as a caregiver and most effectively help the care receiver remain in the least restrictive environment. 2. Increasing caregiver access to community-based services by helping them navigate the service system and providing the information and support necessary for them to access services.
