Provide new or enhanced Family Support Services programs/services to OPWDD eligible individuals who live at home with their families. Proposals must address unserved/underserved individuals and their families with unmet needs who reside within each Region catchment area. Providers of Family Support Services under OPWDD must comply with all applicable NYS OPWDD regulations. Only incorporated not-for-profit agencies or other governmental agencies may apply. Within submitted proposals, OPWDD seeks providers to address those specific priorities (Priority Needs), which have been previously determined through a collaborative process between the DDRFO and District Local FSS Councils. These priorities are based on the 2024 Needs Survey issued by Statewide FSS Committee, Stakeholder feedback, and knowledge of currently available programs within the Region/ District.
