Specifications include, but are not limited to: A. Holly Patterson Extended Care Facility requires numerous software systems to manage resident health. These include clinical systems such as electronic medical records (EMR), electronic medication administration records (eMAR) and electronic treatment administration records (eTAR), financial management and billing software for managing insurance claims, self-pay, and resident accounts, and software for reporting the minimum data set at Medicare/Medicaid-certified facilities. Ideally all of these features should be offered in a single system. The features required include the following: • Point-of-Care: Interface for CNAs and other users to track resident care and health, such as activities of daily living, dehydration risk, skin issues, food intake, weight management, etc. • Clinical/Charting: Full electronic medical records to document procedures and progress notes, input orders, and send/receive care summaries and transition of care documents. •Minimum Data Set (MDS) and Care Planning: For Medicare or Medicaidcertified retirement/nursing communities, the management and reporting of the MDS assessments for each resident are electronically tracked and usually integrated with the point-of-care and charting solutions. • Medications Management: Full eMAR and eTAR, may also include barcode support for scanning of medications, drug-drug and drug-allergy interaction checks, and electronic prescribing. • Patient Scheduling: Scheduling, tracking, and management of resident visits with staff physicians, or at external facilities. • Financials/Billing: Software must offer functionality similar to medical billing software for managing the billing and coding of patient appointments at internal or external facilities, to include a solution for managing resident accounts, including collections of monthly or annual resident fees, and full accounting and management of residents’ finances. Etc. The specific requirements include the following: BASE SYSTEM (IT) Admissions, Discharges, Transfers (ADT) management with pre-admissions and bed holds Census management for auto-calculation of census vs. manual Referral management (integration with Allscripts) to generate reports on admissions by payor source, age, etc. Resident management such as demographics, insurance, contacts, vitals, allergies, etc. Messaging functionality with routing to individuals or groups Reporting with drill down for detailed reports or zoom out for summary reports Face sheets and barcoded wristbands with resident’s allergies, medical alerts and photos Fail-safe system when Internet or system not available Robust system permissions and preferences allow ultimate user management and access control within facility and remotely Dashboard of real-time management and clinical notifications and alerts COMPUTER PHYSICIAN ORDER ENTRY (CPOE) (MEDICINE) Computerized Physician Order Entry (CPOE) integrated into clinical workflow Automated Drug Utilization Review (DUR) with drug interaction validation and duplicate therapy alerts Automated formulary validation for third-party payors such as Medicare Part D formulary Prescription Drug Plans (PDP), state Medicaid and Commercial plans Ability to enter information using structured drop-down options such as route, quantity, measure, frequency, timing, site, duration, priority and additional directions with administration schedule automatically generated based on structure Custom required fields such as diagnosis and justification for off-formulary drugs and DUR alerts Batch order process to facilitate entry of multiple orders on one screen with ability to jump to any component of resident’s chart Custom libraries for standard order sets (e.g., admissions orders) and order triggers (e.g., warfarin requires PT/INR lab) Routing of orders to internal departments or external vendors either electronically or by fax in batches or real-time Automated renewal process for orders based on resident renewal cycles and online physician review & sign-off process Notifications for new, unsigned, and abnormal laboratory results & ability to view and trend laboratory results online; send messages to users with link to laboratory results (requires interface with laboratory vendor) Etc.
