2.1 Overview The Monroe County Office for the Aging (“MCOFA”) is seeking an entity to provide an efficient and cost-effective Depression Screening and Intervention Program (“Program”) for Older Adults, age 60 and over, that reside in Monroe County. Respondent(s) must offer a proposal that will meet the scope of services, qualifications and general description of work activities identified in this Request for Proposals (“RFP”). These services are funded by New York State (NYS) via the Community Services for the Elderly Program (CSE), and augmented by Title IIID federal dollars and local area match. 1. CSE is a New York State funding stream used to provide community-based, supportive services to frail, low-income elderly that need assistance to maintain their independence at home. CSE functions as a New York State block grant program that enables localities to determine specific unmet needs and shape the way the delivery system is organized to respond, providing considerable flexibility within the general parameters of CSE’s intent and goals. 2. Title IIID of the Older Americans Act requires state and area agencies on aging to provide for the highest level of Evidence Based Health Promotion Services as recognized by the Administration for Community Living. This assures that older people within the planning and service aging arenas will have reasonably convenient access to such services. Given the limited amount of funding, Respondent(s) are encouraged to link with existing programs in the aging services delivery system to more effectively meet the desired goals. Respondent(s) must provide a plan on how they can meet the goals and objectives of each community-based direct service they are interested in providing. All plans must include: 1. Narrative describing what the Respondent(s) propose to provide and how they will meet goals and objectives. 2. Detailed staffing plan which includes: a. Description of staff including job duties and qualifications. b. Proposed budgets and staffing pattern plans must provide detail as to specific full time equivalent (FTE) amounts for each person charged to the Agreement, including number of hours per FTE. Respondent(s) must provide proof, when requested, that the allocation of staff for this service complies with the budget. c. Detailed budget outlining cost per unit of service, available match and from where match originated (i.e., foundation funds, other grant that allows funds to be used as a match, fund raising). d. Details on any subcontractors hired to perform services, including: how subcontractor determined; services subcontractor will provide; experience of subcontracted provider, and costs associated with subcontracting, including contracting and monitoring costs of Respondent(s). e. Respondent(s) should indicate whether they could provide additional units of service for the Program if additional funds were made available and if they could provide additional match if they were able to provide additional units of service.
