The Dormitory Authority of the State of New York (DASNY) is seeking qualified vendors in the modular home industry. Interested vendors must meet the qualifications, terms and conditions enumerated below. Background: DASNY on behalf of the Office for People with Developmental Disabilities (OPWDD) is requesting bids to provide a total oftwelve Health Care Community Residences in the State of New York for various Developmental Disabilities Services Offices (DDSO). Each residence will consist of a single story 5,000 GSF with a full basement (full basement by others) and provide eight bedrooms, two full bathrooms, one half bathroom, kitchen, living room, office, laundry room, medical room and an attached 700 SF panelized garage, all as designed and specified by the DASNY consultant. The vendor shall be responsible for delivery of the modular units to the various site locations and for setting the units on foundations provided by others. All modular residences are to be manufactured, delivered and installed in accordance with detailed specifications and construction schedule provide by DASNY. Six Residences will be located in the counties of Broome and Dutchess. Five more Residences are being planned as add alternates. A target for estimating production for the scheduled delivery and installation dates for each of the eleven modular residences will start approximately in late winter of 2011 and end in the spring of 2012 as follows: LocationNumber of Residences a. Taconic DDSO: 1, Plus 2- Add/Alternates The DDSOs are comprised of twenty-one counties in New York State as identified below: a.Broome DDSO: Broome, Chenango, Delaware, Otsego, Tioga and Tompkins b.Taconic DDSO: Columbia, Dutchess, Greene, Putnam and Ulster c.Finger Lakes DDSO: Chemung, Livingston, Monroe, Ontario, Schuyler, Seneca, Steuben, Wayne, Wyoming and Yates. . The successful vendor shall provide: 1.) 100% Performance Bond, Labor and Material Payment Bond 2.) $2 million of General Liability Insurance, Workers Compensation and Disability 3.) Builders Risk Insurance 4.) Transportation Insurance 5.) If required, the capacity to store the modular residences off-site until the site is ready for delivery. 6.) Acceptance of Payment Terms per residence: 10% - upon contract award 20%- upon receipt of certification/permitting by NYSDOS 20%- Verification of that modular residence meets contract drawings 20% - Completion of modular residence in the manufacturers plant 25% - Upon delivery 5% - Upon punch-list completion.
