DOCCS is seeking Chiropractic services at the facilities noted on attachment 1. Services at EACH awarded Lot(s) may also include incarcerated individuals from nearby facilities. These services will begin 60 days after approval by the NYS Comptroller. , Qualifications of Contractor: x The contractor must possess a current, unrestricted license through New York State Board of Chiropractic Examiners and have been trained at an accredited Chiropractic College. x The contractor shall provide evidence that it has maintained an organization capable of performing the work described in this IFB and have been in continuous operation for at least two (2) complete years. x Contractor and/or any member of the contractorís staff who will be providing chiropractic services under the contract, must have two (2) years post license clinical experience, be in good standing, have no disciplinary action in any state, and provide proof of experience upon DOCCS request. x Contractor upon bid must submit resume or Curriculum Vitae (CV), copy of current license and current professional liability insurance. Contractorís Responsibilities: x The contractor will provide exam, treatment plan, and adjustments as determined by the initial exam. The contractor will hold at least one clinic a week for an estimated 52 weeks a year. Clinics are to be agreed upon prior to the start of the contract. If the patient volume requires an additional clinic, alternate days may be requested, or appointments will be carried over to the next week. DOCCS will not hold a clinic with less than ten (10) patients. x Clinic hours will be half day clinics, four (4) hour intervals. Times to be decided upon award and dependent on facility. If all patients are seen before the end of the four (4) hour clinic, the clinic may conclude early. Any changes to clinic hours or days will be mutually agreed upon by both the contractor and DOCCS. An initial exam and treatment plan(s) will be developed based upon clinical criteria. Short term and long- term goals will be established and monitored throughout treatment. If a monthly maintenance care visit is recommended, it may be subject to utilization review. DOCCS reserves the right to investigate or make any inquiry into the capabilities of any bidder to properly perform under any resultant contract.
