Information Providing an individual with information on a one-to-one basis about available resources and opportunities in the community, which enables them to locate and obtain needed resources on their own. Assistance: Assistance provided on a one-to-one basis to caregivers on obtaining access to available services and resources in their community. Includes assisting the caregiver with defining problems, linkages to services, case management, and follow up. Assistance Case Management: Case management assistance to assess caregiver needs (physical, psychological, and social), and to arrange, coordinate, and monitor an optimum package of services to meet the identified needs of the caregiver. Counseling A one-to-one relationship between the caregiver and their assigned care specialist trained to provide supportive counseling. The service is designed to alleviate stress or anxiety and to assist the caregiver in making appropriate choices and solving problems relating to their caregiving roles.
