Specifications include, but are not limited to: A. Services to be provided include but are not limited to: 1. Provide physical therapy services under the supervision of the Head Physical Therapist or designee. 2. Provide appropriate documentation of an individual’s physical therapy treatment plan and progress. 3. Review evaluations, interventions, Life Plans/Comprehensive Functional Assessments (CFA), and residential habilitation programs as requested and/or required by the involved individuals’ service coordinators. 4. Provide in-service education, whenever possible, to the professional and direct care staff of the facility, as related to the physical therapy services and carryover with all parties involved. 5. Provide assistance in the procurement of all physical therapy related equipment and supplies.
