Specifications include, but are not limited to: The Office for People With Developmental Disabilities (OPWDD) oversees the Capital District Developmental Disabilities State Operations Office (DDSOO) and requires qualified Contractors to provide Patient Companion Services for individuals admitted to community hospitals in Albany, Rensselaer, Saratoga, Schenectady, and Warren Counties; 1. The Contractor is responsible to certify that it is fully authorized to do business in the State of New York. 2. It is the Contractor’s responsibility to ensure that any service provided pursuant to this contract complies with all pertinent provisions of Federal, State, and Local statutes, rules, and regulations. 3. The Contractor is responsible for planning, coordinating, and ensuring the quality of all services provided. 4. The Contractor is responsible for adhering to the plan of care established for patients and confirm that it is followed. 5. The Contractor will provide services 24-hours per day, 7-days per week, 365-days per year, as needed by OPWDD. 6. The Contractor will be contacted via telephone when OPWDD requires services. OPWDD will verify the Contractor’s availability at a specific hospital and obtain Patient Companion service coverage. Once coverage is confirmed, site staff will inform the Contractor of specifics regarding the individual and any other pertinent information needed regarding the service to be performed.
