PROPOSAL FORM FOR RFP to be received on or before 4:00 PM EST, August 28, 2023, at the Purchasing Department of SUNY Orange, Orange Hall, Room 203, Middletown, New York 10940.

Proposals must be submitted in a sealed envelope or box plainly marked as to its contents. Required is (1) original proposal and THREE (3) copies. An email submission is allowable in addition to the PAPER requirement.

Proposals must contain a statement of non-collusion as required by Section 103-d of the General Municipal Law. The College reserves the right to reject any or all proposals. The undersigned proposes to furnish the following services, in accordance with the Request for Proposal. The College is a tax exempt organization, do not include taxes in costs.
