Important Note to Vendors:
To supply plastic bottles and caps to Pharmacy for the County of Rockland. Please supply information as to the quantities packed per carton.

Freight - FOB Destination, Prepaid and Allowed

Prices shall be delivered prices.

Quantities are estimated annual usage, quantities ordered may be greater or less than those stated depending on the needs of the Department of Mental Health and Pharmacy.

Samples of approve equal quoted products may need to be evaluated and shall be required to be furnished within seven days of receiving request.

Prices shall be entered as per each bottle. Quoter shall enter in comments or fax their case quantities specifications.

Award shall be to the lowest quote in it's entirety meeting the required specifications.
