Insurance is required, see attached document for Insurance Requirements. Insurance Certificates must state the company name submitting the quote.

Quantities are annual estimated quantities which may be greater or less than those quantities stated depending on the needs of the County. Any minimum requirements must be stated in quote.

Prices quoted shall be F.O.B. Destination, Prepaid and allowed. Prices submitted shall be delivered prices. Any additional cost should be in the unit price for the product being quoted.

Award shall be made in it's entirety meeting the required specifications.
