Unit Prices quoted shall be delivered prices.

Quantities are estimated annual usage. Average orders are 20 - 30 Cases.

Bidder submitting for an approved equal product shall provide manufacturers product brand, manufacturer product number, description and specifications. Samples sent should state Bidders name, RFQ #, Mfg Product # on it. A minimum of twelve bids shall be required for evaluation.

The County of Rockland Department of Hospital prefers Tie Back Bibs, but will evaluate over the head loop bids. You may submit two quotes if you can supply both types. The Hospital will evaluate all bibs and determine the type that is in the patients best interest.
