Summary of Specifications:
It is the intent of The Valley View Center for Nursing Care and Rehabilitation to solicit bids for the purpose of awarding a one (1) year contract for Water and Sewer System Laboratory Analysis for The Valley View Center for Nursing Care and Rehabilitation.

The Valley View Center for Nursing Care and Rehabilitation retains the exclusive right to extend this contract pursuant to renewal option #1 and/or renewal option #2 as provided for in the contract documents. The Valley View Center for Nursing Care and Rehabilitation will issue a written change order exercising said renewal in compliance with “Article 23” of the “Agreement for Vendor Services” (attached).

Please provide a quote for (1) year rates for Water and Sewer System Laboratory Analysis and renewal option #1 and renewal option #2 rates.
