Summary of Specifications:
The comprehensive eye care service must provide evidence of current licensures, certifications and permits throughout the contracted service period. Said licensures, certifications and permits must be in compliance with the New York State Department of Health. The comprehensive eye care service must provide evidence that all personnel assigned to provide optometric services possess current training, licensure and/or certification; including resumes of supervisory personnel. Personnel assigned to provide direct optometric services must supply proof of MMR and HBV immunization/screening, and proof of PPD screening results done within the last six (6) months.
