A. Bid Cover Sheet, Page 1
B. Bid and Bidder’s Affidavit – Pages 13-17 - SIGNED AND NOTARIZED.
C. Form of Bid Bond – Pages 18-22 – SIGNED AND NOTARIZED.
D. Vendor Background Questionnaire – Pages 34-40 - SIGNED AND NOTARIZED.
E. Contract Signature Page – Page 73 and Pages 74 or 75 (whichever applies)
F. Schedules B through E – Pages 76-80 – SIGNED AND NOTARIZED (where required)
G. Certificate of Contractor Registration for prime contractor with the NYSDOL Public Work Contractor
and Subcontractor Registry – failure to submit this Certificate with your bid may result in the
disqualification of your bid.
H. Bid Schedule of Prices (“BSP”) – Itemized, Page 11 & Attachment A, Page 88
Failure to return the BSP with your bid may result in the disqualification of your bid.
These forms must be submitted with your bid IN A SEPARATE SEALED ENVELOPE,
FAILURE TO DO SO MAY RESULT IN THE DISQUALIFICATION OF YOUR BID.
Non-compliance with any of the above bid submission requirements may result in the
disqualification of the bid.
