Summary of Specifications:
Albany County has decided that it will no longer operate a certified home health agency (CHHA). ACHD has established a licensed home care services agency (LHCSA) through which it will provide certain community wellness services, particularly maternal and child health services including ante partum, post partum, new born, and pediatric home visits. ACHD will also continue to provide other home visit programs directly, such as observed therapy for tuberculosis patients, that are public health services. Schedule One offers data describing the current and proposed home care programming. As part of the implementation of this plan, ACHD wishes to identify a qualified party (a) to whom it may sell all rights, title and interest in CHHA assets, and/or alternatively, (b) for those interested, although not required, with whom it may enter into a contractual relationship as a Licensed Home Care Services Agency (LHCSA) after cessation of operation of the ACHD CHHA. Following cessation of its CHHA operations, ACHD may continue to offer services as a LHCSA that are eligible for certain third party coverage that can be billed only by a certified home health agency. ACHD invites interested parties to present approaches that facilitate access to applicable third party coverage to defray the costs of maternal and child health services. Such approaches may include, but not be limited to, subcontract arrangements with a CHHA which could access third-party payments (e.g., Medicaid) subject to applicable law and regulations.
