Specifications include, but are not limited to: Outreach and Engagement •Accept LEAD client referrals from the SFPD•Make initial contact with eligible clients at the point of diversion or social contact referral•Obtain written consent to participate in LEAD.Data Collection and Reporting •Document and maintain demographic and service engagement data for each LEAD client in a comprehensive, secure database.•Maintain case management notes on each client. Provide monthly aggregate reports on LEAD clients to Policy Coordinating Committee that includes the following data: mean age, race/ethnicity, referral type, service engagement and current status.•Enrollment, Assessment and Tracking•Use agreed upon written and electronic Screening and psychosocial intake forms to fully assess and track client's needs and service engagement•Develop and update individualized care plan, including goal setting and treatment planning
