PURPOSE:
The Albuquerque Area Indian Health Service (AAIHS), Santa Fe Indian Health Center (SFIHC) & Satellite Clinics (specified below) has a requirement for two (2) Independent Social Workers, Santa Fe, NM 87505.
STATEMENT OF WORK:
Social Worker Duties:
The Contractor shall provide Optometry Evaluation and Treatment under a non-personal services agreement as a Licensed Independent Social Worker with the following specifications:
- The contractor shall provide Licensed Independent Social Worker non-personal healthcare services with the following specifications:
- Contractor shall provide counseling assistance to individuals and families with psychological or social problems and crisis. Consults with Clinical Director, Chief of Staff, and/or Staff Psychiatrist on management or referral of all difficult or complex problems.
- Contractor shall provide crisis intervention to include assessments, counseling, diagnosis and coordination of care. May include coordinating admission in psychiatric hospital for acute behavioral health intervention.
- Contractor shall provide services for the adolescents and adults, including the geriatric patient. The services provided will meet the unique physical and psychosocial (emotional, spiritual and social-cultural) needs of these patient populations by means of direct care and teaching.
- Contractor shall provide follow-up and aftercare for patients referred to and from other treatment facilities and providers.
- Contractor shall perform ongoing monitoring and evaluation to improve departmental performance and quality of care. Participates in organizational efforts to improve performance through multidisciplinary teams.
- Contractor shall interview people to establish the nature and extent of their problems, helping them work out plans for improving the situation, providing assistance and services, referring them to community resources and other organizations as indicated and assisting them to understand and modify their own patterns of behavior.
- Contractor shall participate in developing treatment programs for patients, and advises and consults with all levels of professional staff concerning behavior changes of clients.
- Contractor shall provide thorough and concise written documentation of evaluations, treatment and case management services, treatment plans, referrals, and correspondence through the Electronic Health Record in a timely manner in accordance to the Santa Fe Indian Health Center HIM Internal Control policy.
- Contractor shall independently diagnoses based on DSM and/or ICD criteria
- Contractor shall participate as a full-time member of the Mental Health team in formulating patient treatment plans and treatment recommendations through contribution of counseling services and diagnostic evaluations, and knowledge of treatment modalities.
- Contractor shall provide consultation to a wide variety of Indian Health Service staff including physicians and other health professionals, paraprofessionals, and mental health clinicians in providing appropriate actions and advice. Provides professional consultation, as directed, to community resources and/or to segments of the organization other than those to which the incumbent is assigned, such as Tribal, State, Federal and private agencies concerned with the welfare of Indian Health Service beneficiaries.
- Contractor shall receive referrals from other health care professionals for specialty consultation and continued management of Behavioral Health conditions.
- Contractor shall refer and coordinate purchased referred care (PRC) services for patients requiring Psychiatric hospital placement, Residential Treatment services or other specialty services pertaining to Behavioral Health.
- Perform routine duties to include providing behavioral health counseling, substance abuse counseling, and related mental health services to individuals eligible for services provided by the IHS at the SFIHC.
- Contractor shall attend department meetings for patient care purposes. Record keeping, release of information and patient confidentiality shall be compliance with HIPAA guidelines. Contractor will document findings in the SFIHC Electronic Health Record (EHR) at the end of patient encounter according to SFIHC policy and procedures, AAAHC Standards, and CMS regulations.
- Contractor will adhere to guidelines as outlined in the Medical Staff Bylaws, IHS Manual Chapter 14, Mental Health Program Standards, Area Mental Health Guidelines and Service Unit department Social Services, and contained in the Manual, Circulars and Social Services Guidelines. Joint Commission guidelines and criteria must be met in all clinic and hospital locations. Must be familiar with professional Social Work and AAAHC and CMS regulations.
- Contractor will adhere to guidelines as outlined in the Medical Staff Bylaws, IHS Manual Chapter 14, Mental Health Program Standards, Area Mental Health Guidelines and Service Unit department Social Services, and contained in the Manual, Circulars and Social Services Guidelines. Joint Commission guidelines and criteria must be met in all clinic and hospital locations. Must be familiar with professional Social Work and AAAHC literature and Federal and State laws and regulations in connection with assignment.
- Contractor shall participate in Quality Assurance measures which include regular peer review and case presentations.
- Contractor must maintain credentialing and privileges at SFIHC.
- The contractor shall be compensated at the agreed contract amount based on invoices submitted and only for services in direct performance of this contract.
- Overtime compensation is not authorized in this contract.
- Training and training expenses are not covered by this contract, unless it is included as; training required by IHS for new technology or as requirement of the IHS and IHS itself has not had the required training to carry out its mission.
- The contract terms and conditions does not allow for accrual of annual or sick leave or other fringe benefits.
Contractor may be authorized to operate a GSA vehicle for healthcare services covered in this contract to travel from SFIHC to satellite clinics to perform Social Work services. Valid Driver’s License is required.

STATEMENT OF OBJECTIVES:
- Locate eligible and qualified Small Business Indian Firms to support IHS procurement needs under NAICS code: 621330 – Offices of Mental Health Practitioners (except Physicians).
- Utilize the information received with future acquisition planning efforts and potential Buy Indian set aside.
HOW TO RESPOND:
This is a sources sought notice under the authority of the Buy Indian Act. The Indian Health Service (IHS) is attempting to locate qualified and eligible Small Business Indian Firms, “Native American Indian Owned and Operated businesses,” that can provide services/supplies to the Indian Health Service. This is a request for information only, this is not a solicitation. Responses to this notice are not considered offers and cannot be accepted by the Government to form a binding contract. Any information submitted by respondents to this notice is strictly voluntary with no obligation or incurred cost by the government.
Provide with response –
- Certification as a Small Business Indian Firm. The government is ONLY interested in Small Business Indian Firms. (See attached Representative Form).
