1. Establish a single point of contact and a suicide assessment phone line that is answered between the hours of 8:00 AM and 4:00 PM Monday through Friday. 2. Arrange for an independently licensed therapist (LPCC, LISW) to meet with the student and his or her parent(s) or caregiver to conduct the suicide assessment. Complete CSSRS by independent licensed clinician. 3. The therapist must have the Parent(s) or Caregiver (and the student if over 14 years of age) sign a release of information that allows follow up with the school. This would generally be with the school counselor or social worker. 4. Utilize an evidence based suicide assessment instrument that measures the students risk for self-harm/suicide. 5. Once the suicide assessment has been completed, the therapist will meet with the family to share the risk level and recommendations and have the parent and student sign off indicating that they were provided this information. 6. Once the family leaves the office the therapist will immediately call the school contact person and notify them of the suicide risk level and recommendations.
