Registered Nurse Services for the Emergency Room Department

This RFQ is for Non-Personal Service Performance Base Contract for (2) Registered Nurses (RN) ON AS NEEDED BASIS in the Emergency Room Department at Northern Navajo Medical Center (NNMC) located in Shiprock, NM. The tasks to be performed are cited in the attached Non-Personal Performance Work Statement (PWS). Period of Performance: upon award for 6 months with possible extension

SHIFT: The shifts are 12 hour rotational (day or night) including weekends and Government Holidays. The Government DOES NOT guarantee a specific shift to any individual(s). It will greatly depend on patient case load. The assignment is through stated dates above or until the position is filled with a permanent direct hire. In such instances, the RN will be released from contract assignment. The Government DOES NOT guarantee that the contractor will work the number of hours ordered. The hours ordered are only estimated.
INITIAL APPOINTMENTS REQUIREMENTS: Education: Degree or diploma from a professional nursing program approved by the legally designated State accrediting agency at the time the program was completed by the applicant.
EXPERIENCE: The RN must have a minimum of 3 years of independent ER trauma clinical experience, including 5 level triage. Needs to have a strong working knowledge of Emergency Nursing, caring for critically-ill, ranging from neonates to geriatrics. Needs to be able to titrate vasopressors, sedation, paralytics and assisting with bedside procedures.
PROFESSIONAL SKILLS: - Trauma/Cardiovascular patients - Anatomy & Physiology of all age groups: neonates to geriatrics - Triage & assess all ages and presentations of symptoms appropriately using 5 level triage algorithm - Proficient in IV cannulation of all age groups
MUST HAVE THE EQUIPMENT SKILLS: - Nihon Khoden monitors - Alaris IVAC pumps - Level 1 Rapid Infuser
Licensure: Must possess and maintain current State licensure while employed with Indian Health Service (IHS). Certifications (MANDATORY): - a valid and unrestricted State Nursing License - a current Basic Life Support (BLS) Certification with American Heart Association - a current Advance Cardiac Life Support (ACLS) Certification with American Heart Association - Pediatric Advance Life Support (PALS) Certification with American Heart Association - Trauma Nurse Core Curriculum (TNCC)
REQUIREMENTS:
1. Return attached Pricing Schedule fill-in with RN profile. Quote (quote(s) submitted will be negotiated). The rate shall be an all-inclusive work-hours, rate of travel, per diem, lodging/housing, transportation, meals, and applicable taxes (i.e. federal, state and local).
2. A complete and signed Certification Statement for Performance Work Statement (PWS) #08-01 Registered Nurse (RN). (See Attached)
3. e-QIP: Required Background Investigation Information (MUST BE LEGIBLE & COMPLETED. DO NOT RE-FORMAT) PLEASE NOTE: UPON COMPLETION OF eQIP, THE SIGNATURE PAGES MUST BE SUBMITTED TO HUMAN RESOURCE DEPARTMENT ON THE FIRST DAY OF ASSIGNMENT INCLUDING TWO FORMS OF IDENTIFICATION.

5. Three (3) current references with active telephone numbers. The references MUST be noted in a separate Word or PDF document and submitted as an attachment.
6. Required to float to other units on an as needed basis.
7. Contractor will not start work until on Purchase Order start date and signed by Contracting Officer
NOTE: Incomplete submission(s) of REQUIREMENTS WILL NOT be reviewed or considered including direct submission to the hospital unit managers. In addition, any profiles received before or after the RFQ dates WILL NOT be accepted.
The government reserves the right to issue a single award or multiple award to the Schedule contractor(s) whose quote(s) represents the best value as defined by FAR 2.101. In determining best value, the following factors will be considered: price, past performance, and qualification(s)/experience of each proposed Registered Nurse candidate.
