Specifications include, but are not limited to: 1. Recruit and enroll health care providers to implement the FVRx program. Ensure community organizations are given written authority from enrolled health care providers to distribute FVRx vouchers or local food boxes. Enrolled health care providers include but are not limited to: Indian Health Services (IHS), Women, Infants, and Children (WIC) clinics, WIC prenatal clinics, Community Health Worker supported organizations, hospitals, family practice clinics, medical centers and/or other clinics. 2. Conduct cooking classes, food demos, nutrition education, and/or provide educational materials to FVRx participants and track participation. 3. Ensure vouchers or local food boxes are redeemed for fresh or frozen produce at participating grocery stores, farmers markets, farm stands, health clinics, or Community Supported Agriculture (CSA) and that vouchers or local food boxes are redeemed for fresh New Mexico grown produce whenever possible. 4. Collect and provide data including, but not limited to, number of enrolled and newly recruited clinics, number of vouchers or local food boxes distributed and redeemed, amount spent by county or tribal community, numbers of people served by county or tribal community, dollar amount of produce purchased from local farmers, and percent of funding spent on local produce. In addition, provide data collected for federal FVRx programs including, but not limited to, demographics, impact indicators, and nutrition program participation. 5. Submit at least one success story with digital photos and testimonials about participant experience with the FVRx program by June 30, 2024, and each awarded year thereafter by June 30. 6. Performance Measures: CONTRACTOR shall substantially perform the following Department of Health priority pursuant to the Department of Health 2021-2023 Strategic Plan: Goal 2: Improve health status for all New Mexicans.
