Contract A Educational Evaluation (LDTC) Professional assessment of: • Diagnose learning disabilities • Provide individual recommendations for student’s educational programs • Determine ability discrepancies Participate in IEP meetings to discuss findings • Participate in IEP meetings to discuss findings Contract B Psychological Evaluation Professional assessment of: • Student’s current cognitive, social, adaptive, and emotional status • Provide individual recommendations for student’s educational programs • Participate in IEP meetings to discuss findings • Utilize current assessment tools Contract C Psychiatric CST Evaluation Professional assessment of: • Student’s current cognitive, social, adaptive, and emotional status • Provide thorough individual recommendations for student educational programs • Participate in IEP meetings to discuss findings Psychiatric Support Services – (Services to be provided upon receipt of referral only) Professional assessment & direct student services of: • Student’s current cognitive, social, adaptive, and emotional status • Provide thorough individual recommendations for student educational programs • Determine ability discrepancies • Participate in IEP meetings to discuss findings when requested • On-site crisis intervention • Ongoing student consultation services after crisis intervention and/or evaluation • Provide individual recommendations to support students in their least restrictive environment • Evaluations must include two follow up office visits
