1. Identify the space and location that will be utilized to provide reentry transitional housing for six individuals (i.e., a single dwelling, shelter, etc.). 2. Identify the number of individuals that can be housed at any given time. 3. Identify how three meals a day will be provided for residents in the house. 4. Provide a detailed budget of costs per day per individual and a breakdown of what is included in the cost. For example, sheets and bedding, household items for common use (dish soap, laundry detergent). 5. Demonstrate experience working with individuals returning home following a period of incarceration in Camden County, New Jersey. 6. Provide all necessary data reports on a format to be determined upon award.
