I. Purpose: To provide case management and connection for eligible residents involved in the Sheltering the Homeless Mentally Ill program. Successful applicants are required to have an active agreement with this service upon proposal submission. II. Scope of Work: Applicants shall describe how they will meet criteria described in Section 8 of this RFP, in addition to the following service components. a. Conduct needs assessments, develop individualized service plans, and overall case management procedures to measure success of service. b. Coordinate referrals to treatment, community-based resources, benefits, and further housing assistance. Include active MOUs. c. Demonstrate length of case management service, maintaining regular contact with clients and community providers. d. Detail how barriers of housing, health, and stability will be addresses e. Maintain client notes, staff DARs to track work performance and submit required monthly reports on the County’s template. III. Staffing & Organization Requirements: 1. Case Manager: One (1) Bachelor-level case manager. • Minimum two years of experience in working with individuals with SMI and knowledgeable in securing housing services and wraparound service connections. • Proposals shall detail the number of weekly and total annual hours the case manager is expected to perform based on past performance of delivering similar services. • Full-time versus part-time hours must be justified. This position will be reimbursed on an hourly basis. Overtime is not reimbursable. • Supervisor: Applicants who include a qualified supervisor to be reimbursed under this service must refer to Section 8 and the Universal Staffing Requirements described herein. o Define responsibilities, check-in frequency, documentation review. 2. Organization: • Adhere to Section 8 and all Universal Requirements described in this RFP for proposal submission. IV. Projections & Performance • Adhere to Section 8 and universal requirements in this RFP. • Include projected number to serve, anticipated duration of case management service, referrals and how connections will be confirmed. • Include KPIs to determine how success will be measured and how staff performance will be monitored. V. Monthly Programmatic Reporting: • Due by the 15th of each month, using the template that will be distributed by the OMHA upon agreement. • DARs required to be included with billing submissions. • Data Collection shall include, but not limited to: oNumber referred, number of enrollments, referrals, confirmed, referral connections, demographics, substance use & mental health brief history. • Challenges & success shall be communicated to the OMHA.
