Specifications include, but are not limited to: 1) Sheltering the Homeless Mentally Ill; 2) Payee and Financial Planning Services; 3) Socialization, Recreation and Support Group Services for Mentally Ill Boarding Home Residents; 4) Wrap-Around Broker Services; 5) Emergency Shelter Placement with Case Management Services. Homeless Mentally Ill Shelter Services: up to $65,000 Residential shelter services to persons without homes who suffer from chronic and recurring mental illness. Agency should have a minimum of three years’ experience servicing this population. Quarterly Report Outline: The total number of individuals served throughout the reporting quarter, including overall tallies per: o Gender, Ethnicity, Age-Range, Previous hometown, if applicable o Referral Source(s) Each type of setting individuals were placed, with the number of individuals corresponding per placement setting Summary demonstrating: o How services were facilitated/ delivered o Needs of clients; reported or observed. o Types of referrals made to supplementary services o The detail of outcomes and the impact of program services during the reporting quarter
