A. Pre-Employment Physicals Pre-employment physical examinations shall be conducted in accordance with this Contract and instructions from the applicable City of Passaic Personnel Representative. Copies of results shall be forwarded to the applicable City of Passaic Personnel Representative. The contractor shall provide medical examinations and related services to persons who are referred by the City and who produce satisfactory photographic identification. Assure that all facets of the pre-employment examination are performed by a duly licensed physician and performed in accordance with professional medical standards. All physicians shall be Board-Certified by the American Medical Association. All stress tests shall be performed by a Board Certified Cardiologist. All pre-employment physicals shall be scheduled within three (3) working days from the date of request. Physician shall make a determination, in the following manner, as to the suitability of the applicant to perform the required duties of his/her position: Applicant can perform essential duties of the assigned position without restriction. Applicant can perform essential duties of assigned position with restriction(s). (Restrictions must be specified). Applicant cannot perform essential duties of the assigned position. The physician shall provide the City of Passaic Personnel Department with a typed report of examination within three (3) working days from the date of examination informing the City if the individual can or cannot physically perform the required duties of his/her position. Physician shall maintain permanent files on each individual examined. Any procedures other than routine pre-employment procedures require prior approval from the appropriate City of Passaic Personnel Representative.
