The contractor shall provide to the Warren County Health Department Public Health Nursing diem nursing personnel (each a “Placed Person” and collectively the “Placed Personnel”), for temporary emergency or per diem staffing of Public Health Nursing as well as other services that may be required from time to time by the County of Warren. 1. Performing Communicable Disease Investigation. 2. Participating in Blood Pressure screenings within the community. 3. Participating in health fairs within the community. 4. Responds to health questions by individuals and/or families during home visits, in clinics or schools or, while given health demonstrations and lectures to community groups to educate the public on preventive health measures. 5. Visits homes of individuals and/or families in the community and gathers initial background information used to assess their health needs and problems to develop a health care plan.
