• The provider must attend and provide a program status update at 75% of all Cumberland Youth Services Advisory Council (YSAC) meetings held during the calendar year. • The provider must attend and participate in monthly YSAC Multi-Disciplinary Team (MDT) Meetings. • The provider must participate in annual program/site monitoring to ensure the program is functioning properly and in compliance with all requirements. • No amendments to the approved funding program design as outlined in the contract may be made without approval by the YSAC with amendment request made to the YSAC Administrator. • As requested, provider agrees to present YSAC a copy of completed audit reports concerning this grant contract. • As a joint effort between the provider and the YSAC, any press or media contact in relation to this contract must be coordinated in advance. • Employee background checks are required for all program staff working directly with youth. The cost for background checks may be incorporated into the program budget and subsequent unit cost (documentation required to be submitted after award but prior to executed contract). • The provider must possess Sexual Abuse & Molestation liability insurance with limits of liability not less than $1,000,000.00 per Abuse and Molestation claim (documentation required to be submitted after award but prior to executed contract). • The applicant must sign and include a copy of the HIPAA Business Associate Agreement (Attachment C) with their proposal submission. This agreement will take effect if the applicant’s proposal is awarded a contract.
